Liability Waiver

I, the undersigned parent or guardian of the athlete named above, acknowledge that participation in the 1st and Goal 365 Camp involves physical activity that carries inherent risks of injury.

I voluntarily assume all risks associated with participation and release 1st and Goal 365, its coaches, staff, volunteers, and host facilities from any and all claims, demands, or causes of action arising out of or related to my athlete's participation in the camp.

I authorize camp staff to seek emergency medical treatment for my athlete if needed. I confirm that the athlete is in good physical condition and able to participate in the camp activities.

I grant permission for photos and video taken at the camp to be used for promotional purposes by 1st and Goal 365.

I have read and accept the liability waiver above on behalf of the athlete.
